A physician referral is required to initiate funded treatment pathways at our Ajax fertility clinic, including Ontario Fertility Program (OFP) funded IVF cycles.

Our standardized referral form captures the clinical history, recent investigations and consent required to open a chart at Ajax Fertility Centre. Family physicians can complete and return the document by fax, email, or via the secure upload on this page.

Secure documentation upload
Family physicians and patients can submit completed referral packages and supporting investigations here. Files are transmitted over an encrypted channel and routed directly to our intake team.
Confidential by design
All documentation is handled under PHIPA-aligned policies, encrypted in transit and stored in our secure patient record system.
Reviewed by clinical staff
Each submission is reviewed by Dr. Abdel-Hadi's team within two business days. We will contact patients directly to schedule a consultation.
What we'll need
Recent bloodwork, semen analysis (if applicable), pelvic ultrasound report, and a completed referral form signed by a Canadian physician.
